    You are welcome here

If you treat Veterans or eligible family members through community care, this hub is for you. Credentials are verified before anything publishes, and nothing about that process is designed to keep you out.

    Before you apply

    What to have ready

    Your identifiers

National Provider Identifier, state license numbers, and board certification details.

    Coverage and screening

Malpractice coverage in force. Exclusion screening runs automatically against OIG LEIE and SAM.gov.

Your media, if you want it

An introduction video, up to five additional videos, and up to twenty photographs. All optional. None may show identifiable patients without written authorization.

    Practice detail

Locations, languages spoken, accessibility features, and which programs you participate in.

    Background screening

Every clinician screens through Checkr before a record publishes, unless Duty Path grants an exemption. You will receive a standalone disclosure and authorize the screen before anything is ordered.

What happens after you apply. If a credential cannot be verified at its primary source, or screening is outstanding, the record stays unpublished and you will be told exactly which item is open. Nothing goes live on an unverified claim.
A record does not disqualify you. Findings are assessed individually against the nature of the offense, the time since, and whether it bears on this work. See background screening for the full process and your rights.

    Who belongs here

This hub is for you if

    Solo and small practice

You practice on your own terms

A single clinician or a small group taking community care referrals. The Listed tier exists so a verified record costs you nothing in production work.

    Multi-site

You run more than one location

Per-location detail, multiple service lines, and the media allowance to show each one.

    New to community care

You are just starting with VA

You do not need an established community care history to hold a record. You need a current license, a National Provider Identifier, and coverage in force.
